Open Rhinoplasty vs. Closed Rhinoplasty Learn About Your Options

There are two primary approaches to cosmetic nasal surgery:

Closed Rhinoplasty Approach

During a closed rhinoplasty, Dr. Lerman will make a small incision hidden inside the nose. This approach results in absolutely no visible postsurgical scarring because all incisions are made in the interior of the nose. However, this approach has limitations. It can be performed to address obstructed airways, minor nasal tip problems, or a bump on the bridge of the nose.

While the closed approach is ideal for certain cases, Dr. Lerman will recommend the open approach if it better addresses your cosmetic and functional concerns. He will determine the best approach during your consultation at our Manhattan or Westchester County offices.

Open Rhinoplasty Approach

In the open rhinoplasty procedure, Dr. Lerman will make a small incision across the columella, the small piece of tissue between the nostrils. This approach offers better access to the nasal bones and cartilage so more significant aesthetic changes can be made. The soft tissues that cover the nose are exposed and gently raised so Dr. Lerman can reshape the nose structure. In addition, this approach allows Dr. Lerman to build up the nose.

Using cartilage harvested from other areas of the body, such as the septum or ear, Dr. Lerman can augment abnormal nasal structures and straighten them to improve symmetry and respiration.

Anesthesia

After administering anesthesia to ensure your complete comfort throughout the duration of your procedure, your board-certified surgeon will begin your surgery.

Incisions

Based on our recommendations and the treatment plan that was developed during your consultation at our New York City offices, Dr. Lerman will create the necessary incisions. These incisions will be as small and discreet as possible while still allowing Dr. Lerman to achieve the best possible results.

Nasal Reshaping

Next, Dr. Lerman will make any necessary alterations to help you achieve your aesthetic goals. This may involve augmenting, reducing, and refining certain areas of the nose, such as the bridge, tip, nostrils, cartilage, or septum.

Sutures

Non-dissolvable or dissolvable sutures may be used on the inside or outside of the nose to close the incisions.

Splint

A splint or internal packing may be placed to aid in healing and contouring after your nose job. Dr. Lerman will instruct you on the proper use and wear time for your splint, so there's no confusion. He will also likely remove it himself on a follow-up visit.

Nonsurgical Rhinoplasty?

Are you unhappy with the appearance of your nose but not yet ready to undergo nasal surgery? Nonsurgical rhinoplasty using injectable fillers is another option.

Often called a liquid nose job, dermal fillers can be used to change the shape of your nose in a procedure that just takes a few minutes.

The filler, usually hyaluronic acid such as Restylane® and JUVÉDERM®, is placed under the skin to create more volume or smoother lines. It can last anywhere from four months to three years, depending on your skin, the filler, and your desired nasal results. Dr. Lerman can carefully target the right areas to:

  • Add volume to the nose
  • Smooth out small bumps in the nose
  • Lift the tip of your nose
  • Make the tip of the nose more prominent

There are two scenarios when injectables would not produce the desired result. Injectables cannot make a nose smaller and, if you want a dramatic change, Dr. Lerman may recommend surgical rhinoplasty. Liquid rhinoplasty is typically only suitable for subtle, yet noticeable, nasal results.
