Dr. Lerman Explains Breast Augmentation Surgery

Dr. Oren Lerman offers two options for enhancing a patient's breast tissue at his New York City office: using breast implants (filled with silicone or saline) or using a patient's body fat. Both surgical techniques can be effective, but patients may be better suited to certain methods given their aesthetic goals. Dr. Lerman can explain the surgical process in more detail during a consultation and help you make the right choice for your needs.

Before & After Reconstruction with Implants

Restore Self-Confidence and Self-Image Following a Mastectomy

By following the link below, you can see images of a patient who underwent a double mastectomy and subsequent breast reconstruction. Dr. Lerman performed the mastectomy using a nipple-sparing technique. He then reconstructed the patient's breasts with silicone "gummy bear" implants. The results of the surgery were natural-looking, maintaining the appearance of the nipples while the breast implants provided the volume necessary to restore her bustline.

"Like no other care I've ever received." Multiple Patients Share Their Stories

Dr. Lerman has helped many breast surgery patients from New York City and throughout the Tri-State. They come to his Upper East Side office because of his education and experience as a cosmetic surgeon and the quality of his results. Breast Augmentation Candidacy

If you are a healthy adult woman with fully developed breasts, but are dissatisfied with the shape and size of your breasts, then you are most likely a good candidate for breast augmentation surgery. Questions you should consider include:

  • Are you physically healthy?
  • Are you self-conscious wearing swimsuits and form-fitting tops?
  • Are you unhappy with the natural shape of your breasts?
  • Have your breasts lost their shape or volume after breastfeeding, weight loss, or as part of the aging process?
  • Is one of your breasts larger or shaped differently than the other?
  • Did your breasts fail to develop normally?

Regardless of your unique situation, breast augmentation is a personal, individualized procedure, and we always stress to our patients that it should only be undertaken for your own health and self-empowerment, and never for the approval or expectations of others.

Before & After A Lift with Implants

A Breast Enhancement Option for Women with Drooping or Sagging Breasts

Many women with sagging breasts think they need a breast augmentation when a breast lift may be better suited to their needs. Breast lift surgery repositions the breasts for a perkier, more youthful appearance. At his offices in Manhattan and Westchester County, Dr. Lerman can perform a breast lift on its own or in combination with breast augmentation.

Understanding Your Breast Implant Options

Saline Implants

Saline implants are silicone shells that are filled with sterile saltwater. When you choose saline implants, Dr. Lerman will insert the implants deflated, and then he will fill them with the saltwater once they are in place. This method often allows Dr. Lerman to make smaller incisions than would be necessary with silicone implants. Also, saline implants are arguably less risky than silicone implants because the saltwater will be naturally absorbed by the body in the event of a rupture. On top of that, saline breast implants are usually more affordable. The biggest drawback to saline breast implants is that they don't feel as natural as silicone breast implants.

Silicone Implants

Silicone breast implants are pre-filled with a silicone gel. Because they are inserted at full size, the incisions required for silicone implants need to be a little larger. However, silicone implants are a popular option because they feel just like natural breast tissue, and they produce the most natural-looking results. The silicone gel in these implants is not biocompatible in the same way that saltwater is, so ruptures of silicone implants (which are rare) potentially pose a higher health risk. However, if you monitor the integrity of your breast implants with periodic checkups, then you'll have little to worry about in terms of future complications.

"Gummy Bear" Implants

"Gummy bear" breast implants are a type of silicone implant made of a more cohesive silicone gel. The silicone gel in these breast implants is form-stable, so you could cut them in half and no liquid would come out (just like a gummy bear). Therefore, there is no chance of these implants leaking. Also, these breast implants are designed to maintain their shape better than traditional silicone breast implants. "Gummy bear" implants do require relatively large incisions, and they are the most costly type of breast implant that Dr. Lerman offers in Manhattan and Westchester County, NY.

How Much Does Breast Augmentation Cost?

The cost of breast augmentation can vary based on the use of implants or fat grafting, whether you choose to combine augmentation with other procedures, and other factors. When you meet with Dr. Lerman at one of his New York offices, he can explain surgery prices and give you an idea of what it will cost to achieve your goals.

The Breast Augmentation Procedure

The breast augmentation procedure can typically be performed in two to three hours at our Manhattan or Westchester offices.
Dr. Lerman can provide a more detailed overview of the surgical procedure during a consultation at one of his New York City offices.

General Anesthesia

In most cases, we administer general anesthesia so you remain completely comfortable and sleep through the procedure.

Incisions

Dr. Lerman will create the incisions around the areola, in the crease in the underside of the breast, or in the armpit.

Placement

Next, a pocket is created so the implant can be positioned in the ideal location for natural-looking results. Once the pocket for each breast is created, the implants are inserted and we can make any adjustments to achieve good symmetry.

Completing Surgery

After the incisions are closed with sutures, we will apply bandages as needed to protect the incision area and reduce swelling. We will also dress you in a surgical bra. The bandages should remain in place until you return for your first post-operative appointment.

What About Capsular Contracture?

Capsular contracture is a condition that can occur when the body forms a significant amount of scar tissue around a breast implant. In extreme circumstances, the scar tissue can start to constrict the breast implant, which can lead to discomfort and a distorted appearance of the breasts.

Capsular contracture is a relatively rare side effect of breast augmentation surgery, but Dr. Lerman does everything he can at his Manhattan practice to prevent it. To begin, he will conduct a very thorough screening during the consultation process to determine if you are at high risk of capsular contracture. Then, during the surgery, he will operate with extreme precision and adhere to the highest sanitation standards, making capsular contracture less likely.

Ultimately, perhaps the best way to avoid capsular contracture is to choose the right implant type, size, and shape for your body. Dr. Lerman will help you select the right breast implants for your needs, so the chances of complications will be as low as possible.

Following our post-op instructions can help patients manage discomfort and reduce the risk of post-surgical complications.

Light Walking

To prevent complications, patients should take short, light walks starting the day after surgery.

Returning to Work

Most patients take about a week off of work after surgery. Those with more strenuous jobs may need to take several weeks off of work to avoid lifting or straining in the treatment area.

Exercise

Patients can slowly work light to moderate exercise into their routine after a few weeks. However, it is important to wait at least six weeks before engaging in strenuous workouts.

Surgical Bra

Patients should wear an approved surgical bra for at least four to six weeks after surgery. The bra should be worn both during the day and at night.

Breasts Settling

It can take up to three months for the breasts to settle and take on a more natural appearance. Patients will continue to see improvement in the position of their breasts for up to a year.
